SECTION E: STUDENT COMMITMENTS & CONDITIONS AGREEMENT

All the information provided on this form is true and complete to the best of my knowledge. If asked by an authorized official, I agree to provide proof of the information that I have given on this form. I realize that, should I fail to provide proof when asked, financial assistance may be denied.
I shall notify the Town of Drayton Valley of any change in my situation such as name, address, or the receipt of other financial assistance.
I shall provide the Town of Drayton Valley with proof of successful completion (e.g. copy of transcript, certificate, or letter from the postsecondary institution) within 60 days of the program/course completion date, failure to do may result in future applications being denied.
I understand and agree that, if granted, my financial award will be disbursed per semester, and that I will be required to reimburse the Town of Drayton Valley in full if I do not successfully complete the associated courses.
I understand and agree that all requested forms and documents must be on file before the Town of Drayton Valley sends funds directly to the post-secondary institute to assist with my tuition costs.

It is very important for our local government and administration, sponsors, and all others interested in the TAB program to understand its effectiveness for you and our community. For this reason, we have partnered with University of Alberta (UofA) researchers who may request your help in understanding and evaluating the TAB program. This could involve participating in an interview, focus group, and/or survey.

The TAB program facilitates local post-secondary learning opportunities for residents of the Town of Drayton Valley and the Brazeau County region. Administrators reserve the right to periodically review and update the program and its funding criteria. Operation of the TAB program is subject to the availability of funding.
